Participating Organisation:
- Asian Association of Chesterfield and North Derbyshire.
Project Objective:
- Work with members of the targeted BME community to increase their understanding of the availability of carers support services.
- Improve the health and wellbeing of BME Carers.
- Increase the understanding of the services provided by Derbyshire Carers Association on the needs of carers from the targeted BME community.
- Increase the understanding of Derbyshire Carers Association & commissioners on the needs of carers from the targeted BME community.
Service Outcomes:
- Improve social inclusion of BME Carers.
- Improve the health and wellbeing of BME Carers.
- Targeted BME Community members have an understanding of the Carers agenda.
- Carers have a better experience of accessing Carer Services.
- Carers have access to information in appropriate community language.
Service Output:
- Continue to support existing identified carers from the targeted BME community.
- Identify new carers from the targeted BME community, give ongoing support and encourage them to register with
their GPs as carers in
partnership with Derbyshire Carers Association
- Encourage existing and new carers to apply for Carers Allowance Entitlement with support from DCA
- Ensure and support existing and new carers from the targeted BME community to be registered for the Carers Emergency Card Scheme
in partnership with Derbyshire Carers Association.
Service Condition:
- The participating organisation must send representative(s) to attend all the bi-monthly Derbyshire County Council's BME planning meetings
to share progress and exchange information with other participating organisations in the BME Carers Project
