Public Health Project on Socal Isolation :

Phase 1 :

  • Identify and contact socially isolated members of your community.
  • Gather baseline information (See form)
  • Organise an event/activity as a starting point
  • Discuss with participants ideas for activities in relation to social prescribing

Phase 2 :

  • Provide additional lunch club/activity once a month to include information from Public Health professionals, healthy activities e.g. walking, chair exercise. These should be based on feedback from participants
  • I joint trip for participants of all groups – Carsington Water, contact with National Trust, Chatsworth.
  • Meeting on September 10th with Bev and Helen from Public Health.
  • Discuss with participants ideas for activities in relation to social prescribing

Phase 3 :

Collection all monitoring to include:

  • Numbers engaged
  • Number who regard themselves as socially isolated
  • Methods used to find these people
  • Number of referrals into services – what was the service, what was the outcome
  • Case Studies:
    • How socially isolated did they feel when they started the project?
    • How isolated do they feel at the end?
    • Have they made new friendships?
    • Do they see these friends outside the project?
    • What difference has it made to their lives?
    • Has their physical or mental health improved and in what way?
